What to Bring to Your First Visit

To help us provide the best care, please bring the following:

  • Your insurance information (if applicable)
  • A valid ID
  • Any medical records or lists of medications that may affect your dental care

We also recommend arriving a few minutes early to complete any necessary paperwork.
